Responsibilities

1. Attend to all personal care needs including assistance with dressing, hygiene, toileting, locomotion, and repositioning as Care Planned, and with attention to resident's appearance. 2. Serve meals and assist residents with meal intake as required; provide, offer, and assist with hydration needs, snacks, and supplements. 3. Observe and report changes in resident's physical, emotional and behavioral symptoms to the Charge Nurse, House Supervisor, or Nurse Coordinator. 4. Respond to resident calls in a timely manner; provide requested services if within CNA scope of practice. 5. Assist Residents with physical needs for locomotion (ambulating or via wheelchair) and positioning to promote comfort, safety and the highest level of function and independence. 6. Maintain accurate and complete documentation of nursing observations and cares, including but not limited to intake/output, food consumption, activity of daily living. Assure Care Worksheets are used - notify nursing if discrepancies are found. 7. Encourage participation and assist residents to/from activities. Provide additional activities for residents and document non-structured activity participation. 8. Leave residents rooms, work area, hallways, utility rooms, and other areas neat and free of clutter. Provide environmental service rounds to minimize odors. Launder or assist residents in laundering personal clothing. Cleaning of personal care items.
