The YC3 Central Navigator (CN) will act as the primary contact for families, youth, and individuals seeking resources and support. The primary objective of this role is to strengthen social connections and provide tangible support. The Central Navigator will cultivate effective professional relationships with YC3 partners, agencies, and York County community members. In collaboration with these partners and agencies, the Central Navigator will work to enhance preventive measures and support for YC3 clients.
Responsibilities
- Client Interaction:
- Works directly with clients in the Community Prevention system.
- Serves as the primary point of contact for all Community Prevention (CP) cases.
- Conducts in-person meetings with clients to evaluate their needs and provide appropriate support.
- Helps clients overcome obstacles by facilitating phone calls, completing forms, and performing other necessary tasks.
- Develops and maintains an understanding of the needs of local individuals, children, and families.
- Works directly with clients in the Community Prevention system.
- Collaboration and Outreach:
- Collaborates with other agencies and utilizes existing prevention infrastructure to enhance support.
- Maintains open communication with community coaches for clients already receiving support.
- Participates in monthly coaches’ meetings to discuss client goals, needs, and next steps.
- Assists in outreach activities to inform agencies and families about the initiative and available services.
- Maintains regular communication regarding service eligibility and planning with other agencies and services.
- Participates in the Coalition, its Board, and sub-committee meetings.
- Manages and enhances a referral system between the Coalition and referral sources (schools, law enforcement, churches, and other human service agencies).
- Collaborates with other agencies and utilizes existing prevention infrastructure to enhance support.
- Coaching Services:
- Provide coaching to clients not currently engaged with community coaching.
- Coaching Services include but are not limited to client interviews, weekly contact and support, and monitoring coaching services for 90 days.

- Recording Keeping:
- Maintains accurate records and enters applications into data base as required by the CP initiative.
- Completes detailed and comprehensive case notes and meeting documentation.
- Compiles, enters, and analyzes program-related data. Prepares and disseminates an annual report.

- Other duties and responsibilities:
- Demonstrates a thorough understanding of York County community services and resources available to a diverse population.
- Knowledgeable and sensitive to issues about trauma and its effects and supports trauma informed changes in service delivery.
- The CN will possess a comprehensive understanding of poverty and the various systems available to support clients experiencing poverty.
- The CN will strive to acquire knowledge about the diverse cultures within York County to ensure they maintain cultural awareness and sensitivity.
- Attends trainings, presentations, and meetings regarding protective factors and family centered practices.
- Administers support funds in accordance with YC3 policies and funding requirements for clients.
- Assists in annual/monthly CP budget planning and adheres to said budget as set by the Director.
- Demonstrates a thorough understanding of York County community services and resources available to a diverse population.
Qualifications
Qualifications: Training, Education, and Experience
- Bachelor’s or equivalent in training or experience preferred.
- Relevant experience may be considered in lieu of education.
- Three years of relevant work or volunteer experience preferred.
Qualifications: Licensure/Certification
- Must have a valid Nebraska driver’s license and provide proof of personal driver’s insurance and the ability to utilize own transportation for work purposes.

- Ability to efficiently carry out YC3’s mission statement and comply with its policies and procedures.
- Ability to establish and maintain effective working relationships with community citizens, co-workers, and the public and to be able to demonstrate cultural awareness and sensitivity.
- Must be able to maintain strict confidentiality of sensitive information in accordance with HIPAA regulations.

- Ability to interpret oral and written instructions and do appropriate follow-up.
- Ability to work independently and as a team member.

- Ability to constructively participate in quality improvement efforts.
- Ability to actively participate in marketing and outreach activities when requested and emergency response events when required.
- Bilingual (English/Spanish) verbal and written skills are preferred but not required.
- Proficiency with MS Office products.
- Demonstrated ability to participate in collaborative activities and work flexible hours as needed.
- Demonstrated experience with delivering directly or assuring the delivery of group presentations, training and/or technical assistance.
- Demonstrated high level of self-direction and an orientation to achieve results.
- Ability to recruit and manage volunteers and work with diverse sectors of the community.
- Ability to advance initiatives from conception through implementation and ongoing quality assurance as a neutral leader focused on the coalition’s desired and prioritized outcomes.
- Requires skill in multi-tasking, meeting deadlines and performing under pressure.
