Home Health RN - Case Manager
Responsibilities
York General Home Health has an opening for a full-time RN. Day Shift 8-4:30 with rotating call weekend every 4-5 weekends. Responsibilities include assessing, implementing, coordinating, evaluating, and documenting comprehensive in-home services for home health clients. Supervises home health aides, and non-licensed caregivers providing client care according to agency procedures and state regulations.
Qualifications
A minimum of two years medical/surgical or equivalent inpatient experience is required. Previous in home care experience is preferred, and must possess current IV skills. Must be able to work independently. Must have proof of current auto insurance and have a valid Nebraska driver's license with reliable means of transportation to arrive at client's residence in inclement weather. Must be licensed as an RN in the state of Nebraska and current BLS certification is required.
