Responsibilities

Essential Duties and Responsibilities Works directly with clients in the Community Response program Accepts referrals calls and forms from parents and partners. Meets with providers/families to assure all intake information is received to assess their needs, goals, and eligibility for other services and supports. Gathers information from incoming referral agencies and families and assesses the needs of the family to help identify and refer to appropriate programs, services, or supports the individual would be eligible to participate in. Utilizes a family centered approach to identify current supports and services to increase protective factors. Is neutral and trusted by families and community partners and is not the same person/staff providing the coordinated care management for Community Response target population. May be first point of contact with a family (self-referral when family is not connected by another care management/referral agency) and will provide fast turn-around and knowledge on community resources and supports. Determines immediate need and if Low Touch assists the family in getting that need met. If High Touch the Central Navigator connects caller/person to coordinated care management for further services/supports. Performs administrative duties pertaining to Community Response Maintains accurate records and enters applications into data base as required by the CR initiative. Administers flex funds in accordance with YC3 policies and funding requirements for families when all other funding options are dissolved, ensuring a timely response to meet their immediate needs. Assists in setting annual/monthly CR budget and adheres to said budget as set by the Director. Maintains positive relationships with partner agencies Assists in conducting outreach activities to inform agencies and families in the area of the initiative and available services resources. Maintains regular communication of service eligibility and plan with other agencies and services. Attends trainings, presentations, and meetings regarding protective factors and family centered practices. Knowledgeable and sensitive to issues about trauma and its effects and supports trauma informed changes in service delivery. Central Navigation should be able to respond to immediate crisis and provide support within 24 hours. Coordinates coaching program with coaching partners. Collaborates with coaching partners when referring clients for coaching services.

Qualifications

Qualifications: Training and Education Bachelor’s or equivalent in training or experience preferred. Relevant experience may be considered in lieu of education. Qualifications: Experience Three years of relevant work or volunteer experience preferred. Qualifications: Licensure/Certification Must have a valid Nebraska driver’s license and provide proof of personal driver’s insurance and the ability to utilize own transportation for work purposes. Qualifications: Knowledge, Skills, and Abilities Ability to work with community organizations and good communication for grant and organizational reporting. A demonstration of sound judgement in assessing situations on the phone without discrimination of persons and families seeking assistance is essential. Willingness to attending training as a life-long learner in their field of practice. Supervisory experience is helpful.
